What to do if your dentist is out-of-network with your employer/group plan?

Review your plan documents to confirm if your plan includes out-of-network services. If you are not sure, contact Bento Concierge at 1-800-734-8484.

If your dentist is not familiar with Bento please share your plan documents with them or contact Bento support. We will work with your dentist to make sure your next appointment is seamless and easy.

Bento Members

If you plan includes out-of-network services,  your dentist will submit an ADA Claim Form directly to Bento. If your out-of-network dentist requires payment at the time of services submit your receipt or statement of services directly to Bento for reimbursement.

Please be prepared to provide your 10-digit Bento Member ID and date of birth.

  1. Download the Bento ADA claim form below.
  2. Fill out the form providing the patients Bento Member ID.
  3. Send it to Bento. Claim forms can be faxed or mailed to Bento.
